Director Of Clinical Services (RN)- Under the Supervision of the Administrator he/she oversees the entire Interdisciplinary Group, which includes RN Case Managers, Team LVNs, Social Services, Chaplian, and Hospice Aides.  Approve work methods that render high quailty care.  Establishes methods for coordination of care by all disciplines.  Provides leadership to the Clinical Team.  Supervises and Teaches in order to improve the Agency.  Qualifications: Currently licensed in the State of Texas or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compace (NLC).  Five years experience as a professional nurse preferred. One year of hospice experence preferred and 1 year of supervisory/management experience preferred. Ability to work in a field setting and exhibited ability to make sound nursing judgements.  Ability to assess patient needs. Effective communications skills.  Computer skills preferred.  Must have and maintain an automobile to be used for work.
Case Manager Registered Nurse (RN)-- Under the supervision of the RN Manager, she/he provides intermittent skilled nursing services; communicates the patient's progress with other disciplines and directs, supervises and instructs non-professional hospice aide staff in the provision of personal care to the patient. Qualifications: Currently licensed in the State of Texas or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compace (NLC).  One year experience as a professional nurse preferred. Ability to work in a field setting and exhibited ability to make sound nursing judgements.  Ability to assess patient needs and formulate individualized patient care plans to meet those needs. Effective communications skills.  Must have and maintain an automobile to be used for work.
Team LVN - Under the supervision of the RN Case Manager, he/she provides intermittent skilled nursing services, communicates the patient's progress to the rest of the team. Qualifications - Currently licensed in the State of Texas or in accordance with the Board of Nurse Examiners rules for Nurse Licesnsure Compace (NLC).  One year of nursing experience preferred.  Ability to work in a field setting and exhibit the ability to make sound nursing judgements.  Effective communications skills.  Must have and maintain an automobile to be used for work.
