Your Rights & Responsibilities

As a home care patient, you have the right to be informed of your rights and responsibilities before the initiation of care/service. when a patient has been judged incompetent, the patient's family or guardian may exercise these rights as described below as they relate to:

RESPECT AND CONFIDENTIALITY

you have the right:

1. To be treated with consideration, respect, and dignity, including the provision of privacy during care.

FINANCIAL ASPECTS OF CARE

you have the right:

1. To be informed of the extent to which payment of the hospice services may be expected from Medicare, Medicaid, or any other payer.

SELF DETERMINATION

you have the right:

1. To refuse all or part of your care/treatment to the extent permitted by law and to be informed of the expected consequences of said action.

Grief Support

What Are They?

Group meetings facilitated by Rev. Mary Lewis, Hospice Chaplain The group meets the second Monday of each month from 7-8pm at 9555 W. Sam Houston Pkwy South at Bissonnet Suite 333, Houston, TX 77099.

What Will The Group Offer?

This group offers an opportunity to gather with others who are grieving. Particpants in group discussion and share the experiences of your loss as well as to hear from those in the group how they are working through their loss.

What Is The Goal Of The Group?

The goal of the group is to provide helpful tools to better understand the grieving process. You will have the opportunity to share with others who have traveled the road you traveled or who may still be struggling on their journey looking for support.

Do I Sign Up In Advance?

No, feel free to come when the group is scheduled to meet. Individual appointments for counseling sessions can be scheduled if the support group meetings are not convenient for your schedule.
